  • Step 2: Select the products you want to order from a list of products set up through AdaptHealth that you are eligible to reorder at this time
  • Step 3: Review and order

You will need your Patient ID, Last Name, Date of Birth, and Phone Number to verify your identity.

After verifying your identity, you will be presented with a list of products that are set up through AdaptHealth and are eligible for reorder at this time. Simply select the items you need.
