AdaptHealth will begin providing durable medical equipment service to Kaiser Permanente members on the following schedule, depending on the region where you live.

Starting in December 2025, Kaiser Permanente will be transitioning from your previous provider to AdaptHealth for home medical equipment and supplies (HME).
The timing of the transition will vary, depending on the area of the country that you live in. The transition will start December 1, 2025, and be completed on April 1, 2026. You will be notified a few weeks in advance of the transition date in your area.

Kaiser Permanente is making this change as part of their commitment to continually provide you with the highest quality care, services, and support. With myAPP by AdaptHealth , you will be able to place Mobility & Home Equipment resupply orders via email, text message, mobile application, or through an online portal. In addition, you will be able to easily view your order history, real-time alerts, confirmation of delivery, and will be able to connect with a patient support representative 24 hours a day.

Yes, wherever possible, AdaptHealth will provide the same brands and models that are currently being provided by your previous provider and we will also provide supplies that work with the equipment you are currently using.
AdaptHealth will make it easy to:
- Order new or replacement equipment and supplies.
- Make co-pays or deductibles to AdaptHealth (ie., making one time or recurring auto-payments)
No, both settings and prescription will be transferred to AdaptHealth as is.
AdaptHealth has a variety of ways to order supplies ranging from online and automated options or directly with an agent by phone. You will receive information directly from AdaptHealth on all available options.

Kaiser Permanente and your previous provider have been working closely with AdaptHealth to assure contact and last shipping location are uploaded and ready for your next supply order. To confirm or change, you can contact AdaptHealth directly.
