I agree that I have not previously received for an Owlet BabySat^TM from any other supplier. I understand that I may be responsible for the out-of-pocket cost, and have the ability to use an HSA, FSA or credit/debit card to purchase this product with a physician issued prescription. I understand that by submitting this order, I am granting permission to bill my insurance for this device and will be responsible for any out-of-pocket fees, copayments or deductibles that may apply to this order. I agree that have 10 (Ten) days to return an unopened package once shipped and understand that I am eligible for a warranty replacement directly from manufacturer should there be an issue with the device.
